In order to complete a request for sports equipment, please fill out the Game Changers New York Partner Organization Application form. A representative from the team will be in contact with you shortly.
Please note the following:
- All sports equipment pick-ups take place at our warehouse, located at 150 Eileen Way, Syosset NY 11791. Please call your point of contact upon arrival.
- We recommend you bring a large vehicle and large bags or boxes to the pick up.
